• End Date: 02/26/2027
• Hours Per Week: 40
• Shift Duration: 8-hour shifts
• Schedule: Monday through Friday, 8:00 AM - 4:30 PM, with rotating weekends
• On-Call / Weekend Requirement: Yes
• Estimated Weekly Pay: $2,120 - $2,230

Job Requirements

• Active Registered Nurse (RN) license
• Minimum of 1 year of nursing experience required
• Home health nursing or home care experience preferred
• Associate degree in nursing or higher
• Professional certification required for the role
• Ability to work independently and manage a home-based patient caseload
• Strong communication, assessment, and time management skills
• Comfortable traveling within the coverage area to complete patient visits
• Helpful experience with wound vacs, IV care, chemo disconnects, and general assessments

Responsibilities

• Provide skilled nursing care to patients in a home health environment
• Complete thorough assessments and develop patient-focused care plans
• Deliver high-quality clinical care during scheduled home visits
• Monitor patient progress and communicate updates as needed
• Support wound care, IV therapy, and other ordered nursing interventions
• Coordinate care with the interdisciplinary team to promote positive outcomes
• Educate patients and families on treatment plans and self-care guidance
• Maintain accurate and timely documentation for all visits and services
• Manage productivity expectations while maintaining excellent patient care
• Participate in rotating weekend coverage and on-call responsibilities as scheduled
