Home Health RN Job Details

• Weekly Estimated Pay: $2,120-$2,230

• Hours: 40 hours per week
• Additional Coverage: 1-2 nights of on-call coverage per week and 1 weekend per month
• Travel: Mileage reimbursement is available between patient visits

Home Health Registered Nurse Requirements

• Active Registered Nurse (RN) license in Pennsylvania
• At least 1 year of professional nursing experience; previous home health nursing experience is strongly preferred
• Associate degree in nursing or higher
• Current CPR certification
• Strong patient assessment, clinical judgment, organization, and patient education skills
• Ability to work independently in patient homes and travel throughout the assigned Luzerne and Lackawanna County service area
• Professional navy-blue scrubs required

Home Health RN Responsibilities

• Perform start-of-care, resumption-of-care, recertification, evaluation, and discharge visits
• Complete comprehensive nursing assessments and develop individualized plans of care
• Provide wound care, Foley catheter care, IV therapy, and other skilled nursing interventions
• Support patients with chronic disease management and changes in health status
• Educate patients and caregivers on medications, treatment plans, symptom management, and safe care at home
• Monitor patient progress and communicate clinically significant findings to the appropriate home health care team
• Document care accurately and ensure timely completion of required clinical records
• Coordinate services and promote continuity of care across the home health team
• Respond to scheduled on-call needs and weekend coverage requirements
