Home Health RN Job Details

• Weekly Estimated Pay: $2,270–$2,390
• Location: Bala Cynwyd, Pennsylvania, with home health visits throughout the surrounding Philadelphia service area
• Schedule: 40 hours per week
• Contract Duration: 13-week home health nursing contract
• Start Date: October 26, 2026
• Patient Visits: Approximately 25–30 visits per week, including start-of-care assessments and revisits

Home Health Registered Nurse Requirements

• At least 2 years of recent, relevant home health care nursing experience
• Active Registered Nurse (RN) license in Pennsylvania
• OASIS experience required
• Epic electronic medical record experience required
• Strong clinical judgment, organization, and communication skills
• Ability to work independently while coordinating care with patients, families, providers, and the interdisciplinary team

Home Health RN Responsibilities

• Provide comprehensive skilled nursing care to patients in their homes
• Complete start-of-care assessments, follow-up home visits, and ongoing patient reassessments
• Perform accurate OASIS assessments and maintain timely clinical documentation in Epic
• Develop and implement individualized home health care plans based on patient needs
• Monitor patient progress, identify changes in condition, and communicate concerns promptly
• Educate patients and caregivers on medications, treatments, disease management, and home safety
• Collaborate with providers and other members of the care team to promote positive patient outcomes
