Home Health Registered Nurse (RN) Case Manager – Ohio Contract Job

Home Health RN Job Details

• Location: Uniontown, Ohio, serving patients throughout Summit, Portage, and Medina counties
• Weekly Estimated Pay: $2,120–$2,230
• Start Date: October 12, 2026
• Contract End Date: January 8, 2027
• Contract Duration: Approximately 13 weeks
• Schedule: 8-hour shifts, approximately 8:00 a.m.–4:30 p.m.; 40 hours per week with 36 hours guaranteed
• On-Call and Weekend Requirement: One on-call week and one weekend during the assignment
• Travel: Mileage reimbursement between patient visits, from the first patient to the last patient
• Patient Care Setting: Home health
• Dress Code: Navy or black scrubs

Home Health Registered Nurse Job Requirements

• Current Registered Nurse (RN) license in the state of Ohio
• At least 1 year of professional Registered Nurse experience
• Associate degree in Nursing or equivalent educational preparation
• Required professional nursing certifications and credentials in good standing
• Strong patient assessment, clinical judgment, documentation, and care coordination skills
• Valid driver’s license and reliable transportation for community-based home health visits
• Previous home health nursing or case management experience preferred
• Experience with Homecare Homebase (HCHB) is a plus

Home Health RN Case Manager Responsibilities

• Serve as the case manager for an assigned home health patient caseload
• Perform comprehensive nursing assessments and develop individualized plans of care
• Provide skilled nursing interventions and monitor patient progress in the home setting
• Coordinate services with physicians, therapists, aides, caregivers, and other members of the interdisciplinary care team
• Educate patients and families about medications, disease management, treatment plans, and home safety
• Identify changes in patient condition and communicate clinical concerns promptly
• Complete accurate and timely clinical documentation in accordance with home health standards
• Support continuity of care through follow-up visits, reassessments, and interdisciplinary collaboration
• Provide on-call and weekend coverage as scheduled
