Join our healthcare team as a Home Health and Hospice Registered Nurse (RN) in St. Albans, Vermont, providing compassionate skilled nursing care to patients in their homes. This 13-week travel nursing contract offers an opportunity to combine independent clinical practice, patient and caregiver education, and interdisciplinary care coordination while helping patients receive safe, personalized care in a familiar environment. Home Health RN Job Details
- Location: St. Albans, Vermont
- Weekly Estimated Pay: $2,500–$2,630
- Assignment Type: 13-week travel nurse contract
- Start Date: November 9, 2026
- End Date: February 8, 2027
- Schedule: Sunday–Thursday, 8:00 a.m.–4:00 p.m.
- Shift Duration: 8-hour day shifts
- Hours: Approximately 40 hours per week
- Additional Coverage: On-call rotation and holiday coverage required
- Practice Setting: Home health and hospice care
Registered Nurse Job Requirements
- Current Registered Nurse (RN) license valid for practice in Vermont
- At least 1 year of professional RN experience; home health or hospice nursing experience is strongly preferred
- Knowledge of the nursing process, evidence-based practice, and applicable state and federal healthcare regulations
- Ability to complete accurate and timely OASIS assessments and electronic clinical documentation
- Experience or willingness to develop proficiency with complex wound care, wound VAC therapy, IV care, and IV maintenance
- Strong patient assessment, care planning, organization, and communication skills
- Ability to work independently in patients’ homes while collaborating effectively with physicians, caregivers, and interdisciplinary healthcare team members
- Commitment to patient advocacy, infection prevention, confidentiality, and high-quality compassionate care
Home Health and Hospice RN Responsibilities
- Assess patient and caregiver needs and develop individualized plans of care in collaboration with patients, families, and the interdisciplinary care team
- Provide skilled nursing interventions and implement medical plans of care safely and effectively
- Serve as case manager for assigned patients, coordinating visit schedules and managing caseloads in accordance with productivity expectations
- Complete OASIS assessments, visit notes, care plans, supervisory documentation, and required state and federal forms accurately and on time
- Provide patient and caregiver education regarding medications, treatment options, symptoms to report, discharge instructions, and available services
- Perform and maintain competency in high-acuity home care skills, including IV therapy, wound VAC care, and complex wound management as clinically indicated
- Evaluate patient responses to treatment and update the plan of care to promote safety, comfort, and positive outcomes
- Collaborate with physicians, physical therapists, social workers, aides, vendors, and other healthcare professionals to coordinate comprehensive home-based care
- Assess the need for home care aide services, coordinate the plan of care, and provide appropriate ongoing supervision
- Coordinate safe discharges and transfers, ensuring that required documentation and patient information are completed
- Participate in case conferences, peer review activities, quality initiatives, and incident reporting as required
- Advocate proactively for patient and family needs while respecting individual goals, preferences, and dignity
