Home Health Hospice RN Job Details
- Weekly Estimated Pay: $2,500–$2,630
- Location: Saint Albans, Vermont
- Assignment Type: 13-week Registered Nurse contract
- Start Date: October 8, 2026
- End Date: January 7, 2027
- Schedule: Monday–Friday, 8:00 a.m.–4:00 p.m.; some weekends, on-call coverage, and holidays required
- Shift Duration: 8-hour day shifts
- Guaranteed Hours: 37 hours per week
- Practice Setting: Home health and hospice care
Home Health and Hospice RN Requirements
- Valid Registered Nurse license eligible for practice in Vermont
- At least 1 year of professional nursing experience; home health or hospice nursing experience strongly preferred
- Working knowledge of the nursing process, OASIS documentation, care planning, and comprehensive clinical assessment
- Ability to provide compassionate end-of-life care and home-based skilled nursing care to patients and families
- Proficiency or willingness to maintain competency in high-acuity home care skills, including IV therapy, wound VAC management, and complex wound care
- Strong communication, organization, clinical documentation, and care coordination skills
- Ability to work independently in the field and participate in weekend, holiday, and on-call coverage as scheduled
Home Health Hospice RN Responsibilities
- Perform comprehensive patient and caregiver assessments and identify ongoing physical, emotional, and supportive care needs.
- Develop, communicate, and regularly update individualized plans of care in collaboration with patients, caregivers, providers, and interdisciplinary team members.
- Serve as case manager for assigned patients, coordinating visit schedules, services, referrals, and follow-up care.
- Provide skilled nursing interventions and implement prescribed medical plans of care safely and effectively.
- Deliver hospice coverage and compassionate support to patients and families during on-call assignments.
- Complete accurate, timely OASIS assessments, visit notes, care records, incident reports, and required clinical documentation.
- Provide patient and caregiver education regarding medications, treatment options, symptom management, discharge instructions, reportable conditions, and available services.
- Monitor patient responses to treatment, evaluate outcomes, and modify care plans when clinically appropriate.
- Collaborate with physicians, therapists, social workers, aides, caregivers, and other care team members to promote safe, coordinated home health and hospice care.
- Delegate and supervise appropriate tasks within the scope of practice while ensuring patient needs are consistently met.
- Coordinate safe patient discharges and transfers, including completion and communication of required information.
- Advocate for patient and family preferences while promoting dignity, comfort, safety, and quality of life.
