Compensation: $34.25-$47.10 per hour based on experience

Position Overview

A leading acute care hospital is seeking an experienced RN Case Manager to support patients and families throughout their healthcare journey. This role focuses on care coordination, discharge planning, resource management, and ensuring safe transitions across the continuum of care.

The RN Case Manager serves as a key member of the interdisciplinary team, collaborating closely with physicians, nursing staff, patients, and families to develop individualized care plans and achieve optimal patient outcomes.

Responsibilities

• Coordinate discharge planning and transitions of care
• Complete psychosocial and care management assessments
• Develop individualized care plans based on patient needs and goals
• Educate patients and families regarding treatment plans, benefits, and available resources
• Collaborate with physicians and interdisciplinary teams to promote safe, efficient care
• Advocate for patients and assist in overcoming barriers to discharge
• Maintain accurate documentation and regulatory compliance

Requirements

• Active RN license
• Minimum of 2 years of recent acute care experience within the last 3 years
• ICU, CCU, Med/Surg, LTAC, or Case Management experience preferred
• Associate Degree in Nursing or higher
• Strong critical thinking, communication, and organizational skills
• Ability to work independently and collaborate effectively with interdisciplinary teams
• Interest in long-term career growth within hospital case management
