Speech-Language Pathologist (SLP) – Home Health

Serving Oakland County & Wayne County|

Your Responsibilities

In this role, you will:

  • Complete speech, language, cognitive-communication, and swallowing assessments

  • Develop individualized care plans tailored to each patient’s goals

  • Provide therapy services based on physician orders and best-practice guidelines

  • Coach patients and caregivers on exercises, techniques, and home programs

  • Document your visits clearly and promptly

  • Partner with nurses, therapists, and physicians to ensure seamless care

  • Promote safety and help reduce avoidable hospitalizations

What You Bring to the Team

  • Master’s degree in Speech-Language Pathology

  • Active SLP license in Michigan (or Educational Limited License with required supervision)

  • At least one year of SLP experience; home health or hospice is a plus

  • Strong communication and organization skills

  • A patient-first, compassionate approach to care

  • Reliable transportation and valid driver’s license
