Speech-Language Pathologist (SLP) – Home HealthServing Oakland County & Wayne County|

Your Responsibilities
In this role, you will:
Complete speech, language, cognitive-communication, and swallowing assessments
Develop individualized care plans tailored to each patient’s goals
Provide therapy services based on physician orders and best-practice guidelines
Coach patients and caregivers on exercises, techniques, and home programs
Document your visits clearly and promptly
Partner with nurses, therapists, and physicians to ensure seamless care
Promote safety and help reduce avoidable hospitalizations
What You Bring to the Team
Master’s degree in Speech-Language Pathology
Active SLP license in Michigan (or Educational Limited License with required supervision)
At least one year of SLP experience; home health or hospice is a plus
Strong communication and organization skills
A patient-first, compassionate approach to care
Reliable transportation and valid driver’s license
