You’ll serve as the primary nurse for an assigned caseload, delivering high-quality care across the home health continuum by:

• Completing start-of-care, routine, and follow-up nursing visits

• Performing comprehensive patient assessments

• Developing and updating individualized plans of care

• Monitoring patient progress and identifying changes in condition

• Communicating with physicians, clinical leaders, and care teams

• Documenting care accurately and timely in the EMR

• Ensuring compliance with state, federal, and agency guidelines

• Maintaining patient privacy and HIPAA compliance

• Active, unrestricted RN license

• At least 1 year of nursing experience in a clinical setting

• Associate’s or Bachelor’s Degree in Nursing preferred

• Home Health or Hospice experience preferred

• Experience with HomeCare HomeBase a plus

• Reliable transportation, valid driver’s license, and auto insurance

• Ability to travel within the service area (approximately 50%)

• Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
