🩺 What You’ll Do

As a Registered Nurse – Home Health, you will:

• Deliver skilled nursing care aligned with Elara Caring’s mission and values
• Manage and coordinate a patient caseload from assessment through plan of care evaluation
• Conduct initial and ongoing patient assessments in the home setting
• Identify physical, psychosocial, environmental, and spiritual patient needs
• Develop, implement, and update individualized plans of care
• Communicate changes in patient condition to physicians, Clinical Supervisors, and care team members
• Collaborate with interdisciplinary team members to ensure quality outcomes
• Maintain accurate documentation and patient confidentiality in compliance with HIPAA

✅ What We’re Looking For

• Current, unrestricted RN license in the state of practice
• At least 1 year of nursing experience in a clinical care setting
• Ability and willingness to travel within the branch coverage area (up to 50%)
• Reliable vehicle, valid driver’s license, and current auto insurance
• Associate or Bachelor’s Degree in Nursing preferred
• Home Health or Hospice experience preferred
• Experience with HomeCare HomeBase preferred

• Sit, Stand, Bend, Lift, Move Intermitting and be able to lift 50 to100 lbs
