🏥 What You’ll Do as a Physical Therapist

As a key member of our home health team, you will:

• Champion Elara Caring’s mission and philosophy of patient-centered care
• Provide direct patient care and evaluate functional needs
• Develop, implement, and adjust customized plans of care
• Work closely with interdisciplinary team members
• Document evaluations, goals, and treatment plans accurately
• Offer direction and leadership to supporting team members
• Serve as a resource and advocate for physical therapy within the community
• Work in a dynamic, physically-demanding environment
• Support patients across your assigned service area

✅ What You’ll Need

• Graduate of an APTA-accredited Physical Therapy program
• Current, unrestricted PT license in the state of practice
• 1+ year of experience in a healthcare setting
• Ability to travel 50% within your coverage area
• Valid driver’s license, reliable vehicle, and current auto insurance
• Ability to bend, lift (50–100 lbs.), and assist with patient transfers
