What You’ll DoManage and coordinate a dedicated caseload of home health patients
Perform comprehensive assessments and develop individualized care plans
Provide 1:1 patient care in the home setting
Collaborate with physicians, therapists, and interdisciplinary team members
Adjust care plans based on patient progress and clinical findings
Build meaningful relationships with patients and families

What You’ll NeedActive, unrestricted RN license
1+ year of clinical nursing experience
Home Health experience preferred but not required
Ability and willingness to travel throughout the Greater Danbury territory
Valid driver’s license, reliable transportation, and current auto insurance
Associate or Bachelor’s degree in Nursing (preferred)
Experience with HomeCare HomeBase (a plus, not required)
Physical Requirements:
Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
