Our Clinical Team Manager (CTM) role is designed for nurses who want to expand their impact, develop leadership skills, and help shape patient care in the home.

We are growing rapidly and looking for passionate nurses who want to grow with us. Through mentorship, structured development programs, and hands-on leadership experience, we invest in helping you become the leader you aspire to be.

• Leading and supporting a team of field clinicians

• Coordinating and overseeing patient care plans

• Coaching and developing nurses and clinical staff

• Ensuring quality care and compliance with clinical standards

• Partnering with leadership to support continued growth in the community

• Current Registered Nurse (RN) license

• Home health and clinical experience

• Experience with home health clinical documentation, including OASIS and electronic medical record systems (Homecare Homebase, etc.)

• Interest in developing leadership and management skills

• Strong communication and collaboration abilities

• Reliable transportation and local travel

• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
