Job Title: Occupational Therapist, Home Health
Schedule: Full-Time | Monday to Friday | 8:00 AM to 5:00 PM

Your Role as an Occupational Therapist
As part of our interdisciplinary team, you will help patients regain independence and improve their quality of life through personalized occupational therapy services in the home setting.
Key Responsibilities
Evaluate patients to determine rehabilitation needs and develop individualized plans of care
Perform initial therapy evaluations and ongoing reassessments according to OASIS and PPS regulatory guidelines
Collaborate with physicians and care teams to ensure quality outcomes and continuity of care
Communicate significant findings or changes in patient condition to the care team
Educate patients and caregivers on activities of daily living using adaptive equipment and therapeutic techniques
Assist patients and caregivers in securing necessary equipment, which may include custom splint fabrication
Provide instruction and support for home therapy programs
Counsel patients and families on therapy goals and care needs
Supervise and provide clinical direction to Certified Occupational Therapy Assistants, Home Health Aides, and other clinical staff
Ensure documentation meets professional standards and regulatory guidelines
This position involves a physically active environment, including patient transfers and movement between patient homes.
What You Will Need
Graduate of an accredited Occupational Therapy program
Current, unrestricted Occupational Therapist license in the state of practice
Minimum one year of experience in a clinical care setting
Ability and willingness to travel within the assigned coverage area
Valid driver’s license, reliable transportation, and current auto insurance
Ability to frequently lift, push, pull, and support up to 50 pounds, including patient transfers and equipment movement
Reporting Structure
This position reports to the Branch Director, Clinical Manager, Clinical Supervisor, or designee.
