š„ What Youāll Do as a Physical Therapist
Youāll help patients maximize their independence, mobility, and quality of life ā right in their own homes. Responsibilities include:
- Delivering direct physical therapy services with excellence
- Evaluating functional needs and updating care plans accordingly
- Designing customized, outcome-based therapy plans
- Collaborating with family members and care teams
- Guiding and supporting therapy assistants
- Maintaining accurate documentation, goals, and progress notes
- Acting as a community and clinical resource
- Working in a physically active, fast-paced care environment
- Safely lifting, transferring, and assisting patients as needed
āļø What Youāll Need
- Graduate of an APTA-accredited PT program
- Unrestricted Indiana PT license
- 1+ year of healthcare experience
- Reliable vehicle, driverās license, and current auto insurance
- Ability to travel within the Mishawaka service region (up to 50%)
- Ability to lift 50ā100 lbs and meet physical demands

Branch Director, Clinical Manager, Clinical Supervisor, or designee.
