🏥 What You’ll Do as a Physical Therapist

You’ll help patients maximize their independence, mobility, and quality of life — right in their own homes. Responsibilities include:

• Delivering direct physical therapy services with excellence
• Evaluating functional needs and updating care plans accordingly
• Designing customized, outcome-based therapy plans
• Collaborating with family members and care teams
• Guiding and supporting therapy assistants
• Maintaining accurate documentation, goals, and progress notes
• Acting as a community and clinical resource
• Working in a physically active, fast-paced care environment
• Safely lifting, transferring, and assisting patients as needed

✔️ What You’ll Need

• Graduate of an APTA-accredited PT program
• Unrestricted Indiana PT license
• 1+ year of healthcare experience
• Reliable vehicle, driver’s license, and current auto insurance
• Ability to travel within the Mishawaka service region (up to 50%)
• Ability to lift 50–100 lbs and meet physical demands

Branch Director, Clinical Manager, Clinical Supervisor, or designee.
