Home Health RN Responsibilities Include:
Perform comprehensive patient assessments and develop individualized plans of care
Manage a designated patient caseload and coordinate care across the interdisciplinary team
Evaluate patients' physical, emotional, social, and environmental needs
Conduct initial assessments, routine visits, and ongoing re-evaluations
Monitor patient progress and identify changes in condition
Collaborate with physicians, therapists, caregivers, and other healthcare professionals to ensure quality outcomes
Educate patients and families on disease management, medications, treatments, and self-care strategies
Update and revise care plans based on patient needs and clinical findings
Maintain accurate and timely documentation in accordance with regulatory and agency standards
Promote patient safety and help prevent avoidable hospitalizations and emergency room visits
Protect patient confidentiality and uphold HIPAA standards at all times
What You'll Bring
Current, unrestricted RN license in the state of practice
Minimum of one (1) year of nursing experience in a clinical care setting
Strong assessment, critical thinking, and communication skills
Ability to work independently while collaborating effectively with a multidisciplinary team
Reliable transportation, valid driver's license, and current auto insurance
Ability and willingness to travel throughout the assigned service area
Ability to sit, stand, bend, lift, and move throughout the workday, including lifting up to 50-100 lbs. when necessary
Qualifications:
Associate or bachelor's degree in nursing
Home Health or Hospice experience preferred
1 Year Clinical RN experience required
Experience with HomeCare HomeBase (HCHB)
