Registered Nurse RN Home Health | 1:1 Patient Care

What you will do

• Manage and coordinate your own patient caseload
• Complete patient assessments and reassessments
• Develop and update individualized care plans
• Perform start of care and follow up visits
• Communicate changes in patient condition with physicians and the care team
• Collaborate with an interdisciplinary team to improve patient outcomes

What you need

• Active RN license
• 1+ year of clinical nursing experience
• Ability to travel within the service area
• Valid driver’s license and reliable transportation
• Home Health or Hospice experience preferred
• HomeCare HomeBase experience preferred
