What You’ll Do

✔ Manage a home health patient caseload
✔ Complete Start of Care and follow-up visits
✔ Perform patient assessments and develop care plans
✔ Communicate with physicians and care team
✔ Document visits and ensure compliance

What You Need

✔ Active RN license (Indiana)
✔ 1+ year of nursing experience
✔ Reliable vehicle, valid driver’s license, and auto insurance
✔ Willingness to travel within the Indianapolis coverage area
✔ Ability to lift, push, or pull up to 50 pounds
