As a Home Health RN, you’ll provide one-on-one care in the patient’s home. No call lights. No multiple patient assignments. Just meaningful care where it matters most.

What You’ll Do

✔ Complete Start of Care and follow-up visits
✔ Assess patients and update care plans
✔ Educate patients and families
✔ Coordinate with physicians and care team
✔ Document visits accurately

What You Need

✔ Active RN license
✔ 1+ year of nursing experience
✔ Reliable transportation and valid driver’s license
✔ Willingness to travel within Lafayette area
✔ Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
