What You’ll Do
- Promote Elara Caring’s mission, philosophy, and policies to ensure exceptional patient care
- Provide direct patient care, including evaluations, treatment planning, and outcome assessments
- Design and implement individualized plans of care tailored to each patient’s needs
- Collaborate with interdisciplinary team members to ensure optimal outcomes
- Direct and support therapy assistants and staff to achieve patient goals
- Maintain accurate and timely documentation, including evaluations, treatment plans, and physician orders
- Serve as a clinical resource and promote physical therapy within the community
- Work in a physically demanding, fast-paced home-care environment
- Perform lifting, transferring, and positioning of patients as required
- May work irregular hours or participate in call rotation, if applicable
What We’re Looking For
- Graduate of an APTA-accredited Physical Therapy program
- Current, unrestricted Physical Therapist license in the state of practice
- Minimum of one (1) year of experience in a healthcare setting
- Willingness and ability to travel up to 50% within the assigned coverage area
- Dependable vehicle, valid driver’s license, and current auto insurance
- Ability to lift, push, pull, and support up to 50 pounds
You will report to the Branch Director, Clinical Manager, Clinical Supervisor, or designee.
