What You Will Do

• Manage and coordinate your patient caseload
• Complete start of care visits and follow-up assessments
• Develop and update individualized care plans
• Monitor patient condition and communicate with physicians
• Educate patients and families on treatment plans
• Maintain accurate clinical documentation
• Support patients throughout their care journey

What You Need

• Current Registered Nurse RN license
• 1 year of clinical nursing experience
• Ability to travel within the branch service area
• Valid driver’s license and acceptable driving record
• Ability to lift, push, pull, or support up to 50 pounds
• Associate or Bachelor’s degree in Nursing preferred
• Home Health or Hospice experience preferred
• HomeCare HomeBase experience preferred
