Physical Therapist – Home Health

We are seeking a compassionate and motivated Physical Therapist to join our Worcester Home Health team. In this role, you will provide personalized, one-on-one therapy to patients in their homes, helping them improve mobility, restore function, reduce fall risks, and achieve greater independence in their daily lives.

You will have the autonomy to manage your schedule and treatment plans while receiving the support of an experienced, collaborative clinical team.

What You’ll Do

As a Home Health Physical Therapist, you will evaluate each patient’s functional abilities and develop an individualized treatment plan designed to improve safety, mobility, strength, and independence.

Your responsibilities will include:

  • Completing comprehensive physical therapy evaluations in patients’ homes

  • Assessing mobility, strength, balance, range of motion, pain, endurance, and functional limitations

  • Developing and implementing individualized plans of care based on each patient’s needs and goals

  • Providing skilled treatment to improve functional mobility, transfers, gait, balance, and overall independence

  • Educating patients, families, and caregivers on therapeutic exercises, fall prevention, home safety, and the proper use of assistive devices

  • Monitoring patient progress and adjusting treatment plans to support optimal outcomes

  • Identifying changes in a patient’s condition and communicating concerns promptly to the physician and interdisciplinary care team

  • Collaborating with nurses, physicians, occupational therapists, speech-language pathologists, therapy assistants, and other healthcare professionals

  • Directing and supervising Physical Therapist Assistants and support staff, when applicable

  • Documenting evaluations, treatments, goals, progress, and changes to the plan of care accurately and on time

  • Maintaining required physician orders, certifications, and signatures

  • Following all agency, professional, safety, infection-control, and regulatory standards

  • Protecting patient privacy and confidentiality in accordance with HIPAA requirements

  • Participating in educational programs, in-service meetings, and professional development opportunities

  • Serving as a clinical resource and promoting the value of physical therapy within the community

  • Performing additional duties and special projects as assigned

What You’ll Need
  • Graduate of a physical therapy program accredited by the Commission on Accreditation in Physical Therapy Education

  • Current, unrestricted Physical Therapist license in Massachusetts

  • Current CPR certification through the American Heart Association or American Red Cross

  • At least one year of Physical Therapist experience in a healthcare setting preferred

  • Home health experience preferred but not required

  • Strong clinical assessment, communication, organization, and documentation skills

  • Ability to work independently and collaborate effectively with an interdisciplinary team

  • Ability to remain calm and use sound clinical judgment during urgent or crisis situations

  • Knowledge of the physical and developmental needs of the patient populations served

  • Reliable transportation, a valid driver’s license, and current automobile insurance

  • Willingness and ability to travel throughout the assigned Worcester territory

  • Ability to meet all agency health-screening and testing requirements

  • Sit, stand, bend, lift and move intermittently and be able to lift 50–100 lbs.
