As a Physical Therapist, you’ll contribute to our success in the following ways:
- Evaluate patients and develop individualized treatment plans.
- Provide physical therapy services in the home setting.
- Monitor patient progress and adjust care plans as needed.
- Collaborate with physicians, caregivers, and the interdisciplinary care team.
- Complete timely and accurate clinical documentation.
- Supervise therapy assistants and support staff, as appropriate.
What is Required?
- Graduate of an American Physical Therapy Association (APTA) approved physical therapy program
- Current, unrestricted PT license valid for the state of work
- Minimum one (1) year of experience in a healthcare facility
- Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment

The salary range for this position is $100,000 - $115,000 annually, based on the company’s good faith estimate at the time of posting.
Actual pay will be determined based on factors such as education, experience, skills, and internal equity.
This role is also eligible for additional compensation, which could include performance-based bonuses, overtime, on-call pay, or other incentive earnings. These components are not guaranteed and depend on individual and/or team performance.
