·         Candidates must be able to start on the listed start date, or no more than one week later
·         RTO must be on the cover page and cannot exceed 7 days. No RTO during the first week of assignment
·         Please list all facilities the candidate is willing to work in. Specify facility name(s), states, or all, if open to any facility in any state
·         All shifts the candidate is willing to work
·         All departments the candidate is experienced and willing to work

Requirement description : ·         Must have a minimum of 24 months ER experience (with recent experience in last 12 months) - [REQUIRED]
·         Life support certifications must be from AHA, ARC, or ASHI - [REQUIRED]
·         No hits on state license to practice - [REQUIRED] [DOCUMENT]
·         RTO cannot exceed 7 days, No RTO during the first week of assignment - [REQUIRED]
·         Candidate driver’s license to verify residency - [REQUIRED]
·         Travel experience - [HIGHLY PREFERRED] 
CERTIFICATION REQUIREMENTS :

• ACLS (AHA)
• PALS (AHA)
• TNCC
• BLS (AHA)
• NIHSS

STATE LICENSE REQUIREMENTS :

• Arizona

ADDITIONAL LICENSE REQUIREMENTS : Must have active state license to practice at time of submission
Weekend Requirements : To be determined by the facility
On Call Requirements : To be determined by the facility
