- Invasive Imaging Experience - [REQUIRED]

- Electrophysiology Experience - [PREFERRED]
- Candidates' Permanent Address (Facility will not consider local candidates - 50-mile radius restriction)
- Work History
- Skills Checklist, current to 1 year, pertinent to the speciality - [REQUIRED] [DOCUMENT]
- 2 Supervisory Reference Forms Completed within the last two years - [REQUIRED] [DOCUMENT]
- LARA and/or Nursys must state Active Michigan/MI License - [REQUIRED] [DOCUMENT]
- Active American Heart Association BLS - [REQUIRED] [DOCUMENT]
- Active American Heart Association ACLS - [REQUIRED] [DOCUMENT]
CERTIFICATION REQUIREMENTS :
- ACLS (AHA)
- BLS (AHA)
STATE LICENSE REQUIREMENTS :
- Michigan
ADDITIONAL LICENSE REQUIREMENTS : Must be active!
Weekend Requirements : Call - 1 weekend every 8 weeks
On Call Requirements : 2 weekdays per month & 1 weekend every 8 weeks
