Description
Job Summary (List Format): - Location & Facility: - 920 Church St N, Concord, NC 28025 - 457 licensed beds - Trauma Level III, Teaching Hospital - Centers of Excellence: Chest Pain, Stroke, Bariatric - Department Details: - High census: typically at 99% capacity - Average caseload: 14-16 patients/day (varies by unit) - Required recent experience: 3-5 years in Acute Care Case Management - Previous travel RN-Case Manager experience required (no first-time travelers) - Patient Population: - Adult Medical/Surgical, Cardiac, Ortho, and possibly Pediatrics - Licenses & Certifications: - BSN required - Active RN license - BLS required - Case Management certification preferred - Required Skills: - EPIC EMR experience required - Experience with patient placement (SNF, IPR, LTAC, Hospice, Home Health, DME) - Familiarity with behavioral health needs, complex/difficult placements, homeless population - Strong care management, assessment, care coordination, and discharge planning skills - Department Support: - Supported by Director, Managers, CMO, UM team (including PA), peer Case Managers, and Social Workers - Technology: - EMR: EPIC (required) - Use of Rover Chat in EPIC and phone - Careport referral management experience preferred - Floating: - Possible to float to any unit as needed - Orientation: - 2-5 days, tailored to experience and comfort level - Scheduling & Shifts: - No weekends, no on-call - Expected to work holidays unless pre-approved - Manager-approved RTO - RN shifts: Five 8-hour shifts, starting at 8:00 am - Other Information: - Scrub color: Navy Blue - Free parking - Supportive, teamwork-oriented department culture - Communicative and supportive management and educators - Seasoned and knowledgeable staff
