    Description

    Job Summary (List Format): Occupational Therapist - Home Health - Setting: Home Health; must reside at least 50 miles from the facility. - Workload: Average 6 patient visits per day; expected daily driving distance is 60-80 miles (mileage reimbursed weekly). - Coverage Area: Northern Charlotte, Huntersville, Cornelius, Davidson, Morrisville, Salisbury. - Experience Required: Minimum 1 year in Home Health; first-time travelers accepted if experienced in Home Health. - Patient Types/Diagnoses: Chronic diseases, orthopedic surgeries (e.g., total hips/knees), falls, general weakness, CHF, COPD, occasional stroke patients. - Licenses/Certifications: - Valid drivers license & proof of insurance - NC State OT license - BLS certification - Required Skills: - ADL/AIDL/endurance training - Adaptive equipment assessment/education - Develop care plans for patient function optimization - Strong clinical communication - Preventing readmissions by recognizing symptoms - Proficiency with HCHB (Homecare Homebase) - Technology/Equipment Used: - EPIC Rover (view only) - Focura (for documentation) - Samsung Note 9 phones - HCHB platform - Support Available: Departmental leadership, scheduler, clinical manager, clinical supervisor, and peer team. - Orientation: 5 days plus New Employee Orientation (NEO). - Scheduling: - No weekends, no on-call, no major holidays - Schedule cycle: 13 weeks - PTO/RTO requires manager approval - Other Information: - Scrub color: Grey - Parking available - Team-oriented department culture
