Description
Social Worker
Position: Social Worker – Pre/Post Liver Transplant Patients
Schedule: 5×8-hour shifts, Monday–Friday
Weekends: No
Job Description:
- Provide social work services to pre- and post-liver transplant patients in an outpatient setting.
- Complete comprehensive psychosocial evaluations for liver transplant candidates and post-transplant patients.
- Verify and assess patient caregivers, including conducting caregiver verification by phone.
- Manage a caseload that may include approximately 10 new patients per week.
- Provide psychosocial support, resources, and care coordination based on patient needs.
- Although primarily stationed in the outpatient department, the Social Worker may conduct evaluations for inpatients depending on departmental needs.
