Job Summary (LPN – Mobile Case Manager, Adult/Geriatric Focus)

- Schedule: Full-time (Monday–Friday, 8:00 a.m.–4:30 p.m., 40 hours/week), no Saturdays
- Location: Riedman Administrative Campus (home base); travel required to patient homes, hospitals (Rochester General, Unity Hospital), and provider offices
- Floating: Yes, to various locations as needed
- Specialty: Adult/Geriatric, Transitions of Care Program
- Patient Caseload: Manage a monthly caseload of ~25–30 patients

Core Responsibilities:
- Support the Transitions of Care program to reduce 30-day readmission rates
- Work under RN Care Manager direction to implement patient care plans
- Perform appointment scheduling and follow-up coordination
- Monitor and document biometric data
- Provide and reinforce patient education (not hands-on care)
- Attend patient visits (home, office, hospital) as needed
- Conduct home visits for education and observation
- Maintain frequent patient contact (telephonic and in-person)
- Connect patients to community resources and services
- Document all activities and patient interactions in the EHR
- Attend and contribute to multidisciplinary care team rounds
- Identify and triage barriers to care or medication adherence
- Manage a rolling caseload, reassessing goals for transition or long-term management

Requirements:
- Active LPN license with minimum 1 year experience (Home Health/Mobile Case Management preferred)
- Basic Life Support (BLS) certification
- Strong communication and documentation skills
- Ability to work independently and flexibly
- Reliable transportation for travel (patients are NOT transported by LPN)
- Experience with CHF, COPD, or complex comorbidities preferred
- Knowledge of community resources and care coordination preferred
- Comfortable with technology (EHR, virtual care team meetings)

Additional Information:
- No hands-on nursing (no direct patient care procedures)
- Participation in 30-day patient management cycles, with potential transition to long-term management
- Local applicants accepted (with lower rates if <75 miles)

- Office space available at administrative building when not in the field
