Job Summary (Social Worker – MiraVista, Holyoke, MA)

- Provide direct and indirect clinical social work services to patients, families, and legal guardians within an inpatient behavioral health or acute care setting.
- Conduct biopsychosocial assessments, develop multidisciplinary treatment plans, and deliver case management, individual and family therapy, and discharge/aftercare planning.
- Collaborate with physicians, nurse practitioners, and direct care staff as an active member of the treatment team to develop, implement, and document patient goals and interventions.
- Assist with insurance utilization review and serve as a liaison between patients, families, the hospital, and community agencies for care coordination.
- Apply advanced knowledge of behavioral health treatment and family system modalities for patients with psychiatric illnesses, including risk and safety assessment.
- Utilize Electronic Health Record systems for documentation; experience with Medsphere/Vista preferred.
- Obtain Child & Adolescent Needs & Strengths (CANS) Certification within 30 days of hire.
- Work full-time (40 hours/week), day shift, with no weekend shifts required.
- Bilingual (English/Spanish) skills preferred.

Qualifications:
- LCSW, LICSW, or LMHC (or license eligibility) required.
- Clinical Master’s Degree required.
- Prior experience in inpatient behavioral health or acute care settings preferred.
- Sufficient computer skills for electronic documentation.
- Demonstrated knowledge of community resources and aftercare support systems.
