About the Opportunity

We are seeking a Registered Nurse (RN) to provide skilled nursing care to patients in their homes. This role focuses on patient assessment, care coordination, education, and promoting safe recovery in the home environment.

Opportunities are available for both local and travel contracts.

What You’ll Do

Conduct in-home patient assessments and visits
Provide skilled nursing care including wound care, medication management, and patient education
Coordinate care with physicians and interdisciplinary teams
Accurately document visits and patient progress
Support patients and families with care plans and safety education

Qualifications

Active (or pending) RN license
Graduate of an accredited nursing program
Home Health experience preferred
OASIS experience preferred for some assignments
