About the Opportunity
We are seeking a licensed Physical Therapist (PT) to provide skilled home health services to patients in their homes. This role focuses on improving mobility, safety, and independence while supporting patients in their natural living environment.
Opportunities are available for travel and local contracts. New graduate PTs may be considered with appropriate support.
What You’ll Do
- Perform in-home evaluations and develop individualized plans of care
- Provide skilled physical therapy treatments focused on mobility and safety
- Educate patients and caregivers on exercises and fall prevention
- Coordinate care with physicians and interdisciplinary teams
- Complete accurate and timely documentation
Qualifications
- Doctorate or Master’s Degree in Physical Therapy
- Active (or pending) state PT license
- Home Health experience preferred, not required
- Strong time management and communication skills
