Setting: Home health/hospice (patients in their place of residence)
Pay Range: $75.05 (hourly)
Schedule: Variable shifts (work flexible schedule; travel locally)

Requirements:

• 1-2 years years of experience as a RN

• Qualified applicants MUST have at least 2 years years of experience in Home Health
• Qualified applicants MUST have OASIS experience, Homecare Homebase software experience, current state license as a Registered Nurse, CHPN certification, proof of current CPR certification (required), a current driver’s license, and must have reliable transportation and insurance; knowledge of venipuncture and IV care; competence with stage measurement/treatment of surgical wounds and decubiti, inhalation therapy, phlebotomy for outpatient lab testing, bowel and/or bladder training programs; competence with indwelling catheters and drains, safe use of durable medical equipment in the home setting, and safe use of restraints under physician orders.

• CPR certification is Preferred

• Bachelor of Science in Nursing (preferred)

• Valid RN license

• Be willing to obtain State licensure

Responsibilities: As a healthcare professional in a clinical setting (home health/hospice), you’ll oversee skilled nursing care and coordinate continuity of care.

• Assess home health and/or hospice patients in stable and emergent situations, identifying physical, psychosocial, and environmental needs through documentation, clinical record review, case conference, and team reports.

• Develop, implement, document, and evaluate the Plan of Care (POC) while managing OASIS documentation and coordinating medication administration, pain reassessment, and infection control practices (OSHA) to support quality outcomes.

• Serve as a liaison to patients, families, interdisciplinary team members, physicians, and referring agencies; perform on-call responsibilities, attend interdisciplinary team meetings, and arrange referrals as needed.
