Responsibilities:
- Provide comprehensive home health nursing care to patients in accordance with physician orders and agency protocols.
- Conduct patient assessments, home safety evaluations, and care planning.
- Document patient progress, interventions, and outcomes accurately using HCHB system.
- Educate patients and family members on disease management, medication, and treatment plans.
- Collaborate with interdisciplinary teams to ensure optimal patient care.
