Job Details:

Responsibilities:
- Manage discharge planning and utilization review processes within the hospital setting.
- Collaborate with interdisciplinary teams to develop comprehensive patient care plans.
- Document patient progress and coordination efforts accurately using Midas, Meditech, or Well Sky systems.
- Facilitate efficient patient flow and ensure timely discharges.
- Provide education and support to patients and families regarding care plans and post-discharge needs.
