Responsibilities:

• Conduct thorough assessments of home health patients in stable and emergent situations to identify physical, psychosocial, and environmental needs.
• Develop, implement, document, and evaluate the Plan of Care (POC) to ensure quality and seamless care delivery.
• Gather comprehensive data on patient health factors and integrate findings into the POC.
• Maintain accurate, up-to-date patient records reflecting visits, contacts, problems, plans, actions, and goals.
• Follow infection control protocols to protect staff and patients, including OSHA guidelines.
• Educate patients and families on care, medication, diet, safety, and self-care practices, adapting teaching to cultural and age-specific needs.
• Supervise and direct HHA/STNA/LVN staff to ensure quality care and continuity.
• Assess and manage pain, utilizing appropriate techniques and patient education.
• Coordinate referrals to external agencies as needed.
• Monitor cases to ensure compliance with third-party payer requirements.
• Perform on-call duties and provide emergency services as assigned.
• Communicate effectively with patients, families, team members, physicians, and other healthcare professionals.
• Attend interdisciplinary team meetings.
• Perform other duties as assigned.
