Pay Range: 2022.15 - 2123.26

Contract Length: 13 weeks.

Candidate Type: Traveler

Requirements:
Current state license as a Registered Nurse; CHPN certification; Proof of current CPR certification; Current driver's license; Must have reliable transportation and insurance; 1-2 years experience as a Registered Nurse in a clinical care setting; 2+ years Home health experience.

Responsibilities:

• Assess home health and hospice patients in stable and emergent situations to identify physical, psychosocial, and environmental needs.
• Implement, develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
• Educate patients and/or their families regarding care, diet, medications, home safety, etc., using appropriate teaching methods.
• Provide supervision and clinical guidance to HHA/STNA/LVN to ensure quality service.
• Perform ongoing assessment and management of patient pain, including education of patients and families about pain management.
