Job Title: PT: Home Health

Location: Gainesville, FL

Setting: Home Health

Pay Range: $2800-3000/weekly

Schedule: 8-Hour Days (on-call as needed)

Contract Length: 13 Weeks

Candidate Type: Local and traveler friendly

Requirements:

2+ years of experience as a PT
Qualified applicants MUST have at least 1 year of experience in Home Health
Qualified applicants MUST have an active Physical Therapist license
Be willing to obtain Florida or compact licensure

Responsibilities:

As a healthcare professional in a Home Health setting, you will:

Deliver one-on-one, high-touch care to patients in their homes, including skilled assessments and care plan creation (PT).
Collaborate with families and interdisciplinary teams to optimize patient outcomes, utilizing your expertise in functional mobility and rehabilitation.
Perform evaluations using industry-standard procedures, ensuring compliance with state and federal regulations.
