Job Details:
Job Title: RN (Registered Nurse)
Location: West Bronx, NY
Setting: Hospital
Pay Range: 2492.48 - 2617.10
Schedule: 8:00 AM - 4:30 PM
Contract Length: 13 weeks
Candidate Type: Traveler
Requirements: Must have 1 year of inpatient case management experience
Responsibilities:
- Provide discharge planning and referrals for inpatient hospital patients.
- Collaborate with interdisciplinary teams, including physicians and social workers.
- Manage a caseload of at least 20 patients, including discharge to home, community, or SNF.
- Obtain housing, shelter packets, and specialized referrals such as Hemodialysis and Wound Vac.
- Utilize electronic medical records (Allscripts) to document patient progress and discharge plans.
