Job Details:

Job Title: CWHH-Physical Therapist

Location: Richmond, VA

Setting: Home Health

Pay Range: 2530.40 - 2656.92

Schedule: Discuss with recruiter.

Contract Length: 13 weeks

Candidate Type: Traveler

Requirements: Current State License as a Physical Therapist; Proof of current CPR certification; Current driver's license. Must have reliable transportation and insurance.

Responsibilities:

• Perform clinical assessments to identify patient needs and appropriateness of therapy.
• Document, prepare/review, and complete a Plan of Care (POC), progress notes, phone orders, and other clinical records; revise POC based on evaluation.
• Collaborate with physicians and interdisciplinary team to plan and provide treatment for neuromuscular, neurological, and orthopedic disorders.
• Assist and instruct patients and families in transfer techniques and use of equipment such as walkers, wheelchairs, canes, braces, prosthetic devices, and appliances.
• Provide education to patients and families regarding therapeutic procedures tailored to age, condition, culture, and learning readiness.
• Oversee other team members in patient care activities as appropriate.
• Apply infection control concepts and universal precautions during patient care activities.
• Accept clinical assignments consistent with education and competence.
• Perform other related duties as assigned.
