Job Title: PT Home Health Therapy/Rehabilitation
Location: Indianapolis, IN
Setting: Home Health
Pay Range: 2228.54 - 2339.97
Schedule: 08:00 - 17:00
Contract Length: 13 weeks
Candidate Type: Traveler
Requirements: Must have recent Home Health experience; adult home health experience; minimum 1 full year; CPR; BLS; willing to travel up to 75 miles; valid license in hand; reliable transportation and agency-required liability insurance
Responsibilities:
- Provide home health therapy to adult patients within the designated territory.
- Manage an average of 5-6 patients per day, performing start of care and follow-up visits.
- Document patient visits accurately using HCHB and PointCare EMR systems.
- Travel up to 75 miles from the home clinic, with mileage reimbursed per IRS standards.
- Attend periodic office meetings and on-call rotations as required.
