Schedule: Weekend rotation required, 40-hour orientation, flexible holiday coverage

Requirements:

• Active California RN license (pending license accepted if active by start date)
• Minimum 2 years of acute care Case Manager experience performing care coordination
• Previous travel experience required
• Proficiency with Epic electronic medical records system
• Experience with patient assessments, discharge planning, and family meetings
• Knowledge of HMO/IPA processes, advance directives, and community resources
• Competent in arranging durable medical equipment, skilled nursing facilities, and ambulance transport independently
• Trauma Level I or II experience preferred
