Schedule: Weekend rotation required to meet department needs; 40-hour orientation

Requirements:

• Active California RN license required by start date (pending licenses accepted during application process)
• Minimum 2 years acute care hospital experience as a Case Manager
• Demonstrated experience in care coordination and discharge planning within a hospital setting
• Epic charting system experience required
• Proficiency with InterQual admission criteria, DRG classification, and CMS guidelines
• Competency arranging durable medical equipment, skilled nursing facility placement, and ambulance transport independently
• Knowledge of HMOs, IPAs, advance directives, POLST, and protective services processes
• Strong Microsoft Office skills and computerized documentation proficiency
• Experience facilitating family meetings and developing comprehensive treatment plans
