Schedule: Full-time with weekend rotation to meet department needs; 1:20 patient ratio

Requirements:

• Active California RN license (pending licenses accepted if active by start date)
• Minimum 2 years of acute care hospital Case Manager experience
• Proven experience in Care Coordination and Discharge Planning in a hospital setting
• Proficiency with InterQual Criteria and DRG (Diagnosis Related Groups)
• Experience with Epic charting system required
• Competency in arranging DME, SNF, and ambulance transport independently
• Knowledge of HMOs, IPAs, and CMS guidelines
• Proficient in Microsoft applications
• Experience with complex discharge planning including patients with mental health history, substance abuse treatment needs, homelessness, and advance directives
• Ability to organize and participate in family meetings and treatment plan development
• First-time travelers welcome (subject to clinical screening)
