Location: Anchorage, Alaska
Compensation: Details provided to qualified applicants
Schedule: 5x8 Days, Monday through Friday business hours with every other weekend rotation. Shift times vary between 0700-1530, 0730-1600, 0800-1630, or 0830-1700. 72 guaranteed hours per 2-week period.

• Manage a caseload of approximately 50 patients focused on discharge planning and care coordination across all age groups
• Utilize Epic EMR system in a collaborative environment with full interdisciplinary support including IV teams, pharmacy, social services, and more
• 13-week assignment with 24 hours of orientation and structured onboarding
• Free parking provided at the facility
• First-time travelers accepted with at least 1 year of acute care RN experience

Requirements:

• Active Alaska RN license in hand required
• Current BLS certification required; ACM (Accredited Case Manager) certification strongly preferred
• Minimum 1 year of acute care hospital experience required
• Epic EMR experience required
• Must reside at least 50 miles from the facility (travel assignment only)
• Willingness to float within case management services as needed
• Ability to work 2 out of 3 major holidays (Thanksgiving, Christmas, New Year)
• Familiarity with Milliman Guidelines, CMS regulations, and HIPAA compliance
• Experience with care coordination, discharge planning, DME ordering, and plan of care development
• Microsoft Office and Teams experience preferred
• Hepatitis B vaccination required (titers or declination not accepted)
