Description
Start Date: ASAP
Location: Anchorage, Alaska
Compensation: Details provided to qualified applicants
Schedule: 5x8 Days (shift times between 0700-1700), guaranteed 72 hours per 2 weeks, includes alternating weekends

- Case management focused on discharge planning across all patient age groups including neonates, pediatrics, adults, and geriatrics
- Epic electronic medical record system with comprehensive interdisciplinary support teams
- 24 hours of orientation provided with no on-call requirements
- 13-week assignment with opportunity to extend up to one calendar year
- Daily census of approximately 324 patients in a 407-bed acute care hospital
Requirements:
- Alaska RN license in hand (required for offer)
- BLS certification (required)
- ACM certification (preferred)
- Minimum 1 year of acute care hospital experience required
- Epic EMR experience required
- Acute care case management or discharge planning experience
- Must reside at least 50 miles from the facility
- Knowledge of CMS guidelines, Milliman Guidelines, and HIPAA compliance
- Willingness to float within case manager services to meet facility needs
- Availability to work 2 of 3 major holidays (Thanksgiving, Christmas, New Year)
