Schedule: Days, 1:20 patient ratio, weekend rotation required to meet department needs

Highlights:

• Work in an acute care facility with a diverse patient population including adolescents, adults, and geriatrics

• Epic charting system utilized throughout the facility

Requirements:

• Active California RN license (pending licenses accepted but must be active by start date)
• Minimum 2 years of acute care hospital case manager experience required
• Proficiency with InterQual Criteria and DRG (Diagnosis Related Groups)
• Demonstrated experience in care coordination, discharge planning, and plan of care development
• Epic EMR experience required
• Competency in arranging DME, SNF, and ambulance transport independently
• Knowledge of HMOs, IPAs, CMS guidelines, and experience with diverse patient needs including mental health, homelessness, and substance abuse treatment coordination
• Proficient in Microsoft applications and computerized documentation
• First-time travelers accepted with clinical screening
• Minimum of 2 supervisory references covering 1 year of comparable experience within the last 3 years
