Schedule: Monday through Friday, 8:00 AM – 5:00 PM

Requirements:

• Active Indiana RN license or multi-state RN license
• Current BLS certification through the American Heart Association
• Minimum 2 years of recent RN Case Management experience
• Experience in LTAC (Long-Term Acute Care) or STACH (Short-Term Acute Care Hospital) settings required
• Proficiency with MCG criteria and discharge planning across acute care environments
• Demonstrated ability to collaborate effectively with interdisciplinary care teams
