Requirements:

• Active Registered Nurse (RN) license in Oregon
• Home health or community health nursing experience preferred
• Strong case management and organizational skills
• Ability to work independently and collaboratively with multidisciplinary teams
• Valid driver's license and reliable transportation for home visits
