Location: Charlotte, North Carolina

Schedule: 8-hour day shifts, 8:30 AM – 5:00 PM, 40 hours per week, Monday through Friday

Highlights:

• Work in a skilled nursing facility within a hospital setting, providing short-term rehabilitation care for residents
• Manage MDS assessments, care plan development, utilization review, and clinical documentation improvement
• Collaborate with interdisciplinary team including PT/OT, social services, pharmacy, and rapid response teams
• Patient ratio of 1:12 with average daily census of 9 residents
• Epic EMR system with Pyxis medication management
• 13-week contract with no call, no float, and no weekend requirements

Requirements:

• Bachelor of Science in Nursing (BSN) required
• Minimum 3 years of RN experience required
• Current MDS Coordinator experience within the last 3 years required (hands-on assessment completion, not management role)
• North Carolina RN license or compact license required at start
• BLS certification required
• Epic charting system experience required
• Long-term acute care or skilled nursing facility experience required
• Experience with managed care, utilization review, care coordination, discharge planning, clinical documentation improvement, and CMS regulations
• Knowledge of ICD-10 coding, CPT coding and billing, and Medicare/Medicaid guidelines
• Flu vaccine required (medical/religious exemptions accepted)
• First-time travelers accepted
